How to Get Vaccinated
Walk-ins: Bring a photo ID and any insurance cards. You’ll complete a consent form on site. Most insurance plans cover the flu vaccine; some do not cover the administration fee.
Before Your Visit
Please review our Flu Vaccine Information Sheet and HIPAA Privacy Notice. You’ll be asked to sign a consent form verifying you’ve read both. You may also print the Flu Consent Form, complete it at home, and bring it with you.
On the Day of Your Vaccination
When you come to the pharmacy, you’ll read the Vaccine Information Sheet, complete consent, and provide insurance information.
After Vaccine Adminstration
Please remain in the store for at least 10 minutes so we can monitor for any immediate side effects.
